The process begins with scheduling a family meeting where we will take the time to get to know each other, describe the steps to ensure your child receives the appropriate service and provide an overview of our day habilitation program

Admission procedures

Upon receipt of completed application our admission professional will schedule an intake interview. The purpose of the intake is to gather relevant background, clinical and medical information to determine whether the program can appropriately meet individual's needs.

Coordination with Individual's Support Coordinator 

The program director or designee will consult with the applicant's Support Coordinator to review additional information, including the Person Centered Planning Tool (PCPT), Individualized Service Plan (ISP), and any clinical and behavioral documentation relevant to service planning.

Optional Trial

Prior to an admission decision the applicant may be offered a trial program participation.

Parent Support

Abcare believes in a holistic approach where we are all essential parts of the team. Our clinical lead will work with parents, and other professionals such as Speech therapists, Occupational therapists and The program participant's physicians to ensure their needs are met. We provide regular in-person parent meetings, caregiver support group and education to allow you better support your child.

Continuous Progress Evaluation and Re-Assessments

We assess our client's progress toward all key goals. Based on these evaluations, we make necessary adjustments to the program plan to optimize outcomes and support our client's success.
